The Graduate School requires this form, including the CV, and (for Full or Associate appointment only) the dean’s endorsement of your request. This form should only be initiated by UA faculty/staff.
Current/up-to-date CV should include:
Publications and conference presentations
Current employment and title.
Note: For new UA hires – you cannot use the CV submitted during the interview process, as it would not show current faculty status at UA.
If needed, review Graduate Faculty qualification policies outlined in Appendix J of the Faculty Handbook.

Full May chair thesis or dissertation committees, serve on committees, and teach grad classes.
